Looking for an LGBTQ-affirming doctor in Portugal or Spain? Here's what to actually look for, beyond a rainbow flag in the waiting room.

A rainbow sticker in the window tells you a clinic wants your business. It doesn't tell you whether the provider inside actually knows how to care for LGBTQ patients well. Here's what's worth checking instead.

Ask directly about experience, not just willingness. "Are you comfortable treating LGBTQ patients" gets a yes from almost anyone. "How many transgender or LGBTQ patients do you currently see, and what does that care typically involve" gets you a real answer.

Check how intake actually works. Affirming practices usually have a way to record a chosen name and pronouns that shows up everywhere, not just on a sticky note for the front desk.

Ask how they'd handle continuity, not just a first visit. If you're mid-treatment for anything, including hormone therapy, ask specifically how they'd take over that care and coordinate prescriptions, rather than assuming it will just work itself out.

Look past the clinic to the system around it. A genuinely affirming provider is comfortable saying when something is outside their expertise and referring you onward, rather than guessing.

Use community sources, not just search results. LGBTQ organizations and community networks in Portugal and Spain often have more current, honest information about specific providers than a general web search does.

One country-specific note worth knowing: because Portugal's legal framework for trans-specific care is currently in flux, some families living there are also looking at providers in Spain for that particular piece of care, even while the rest of their healthcare stays local. If that's your situation, it's worth widening the search on purpose rather than assuming your usual doctor covers it.

Vetting all of this yourself is possible, but it's also exactly the kind of legwork a good healthcare navigation service exists to shortcut.
