Most Americans arrive in Portugal or Spain with exactly the insurance their visa application required, and assume it covers them once they land. It doesn't, and the gap between the two catches people at the worst possible moment.
Travel insurance gets you in the door. Portugal's visa application process typically accepts travel insurance with medical coverage to issue the initial visa. It's built for short stays, not for living somewhere.
Long-term coverage is a separate requirement. In Portugal, AIMA requires comprehensive, ongoing health insurance before you submit your residence permit application; travel insurance generally isn't accepted at that stage. In Spain, most long-stay visas, including the non-lucrative visa, require full private health insurance with no co-pays, since non-working residents don't automatically qualify for the public system.
Travel policies often aren't accepted at the doctor's office. This is the detail that surprises people most: even a valid travel insurance policy may not be accepted for direct billing at physicians' offices once you're a resident rather than a visitor.
Public healthcare comes later, and it isn't automatic. In Portugal, SNS registration typically becomes available once you hold your residence permit and begin social security contributions; you register at a local health center with your permit, NIF, and proof of address. In Spain, gaining public system access as a non-working resident may require the convenio especial — a paid-in arrangement — rather than automatic enrollment.
The takeaway: the insurance that got you your visa is not the insurance that gets you through your first year. Confirm exactly when you need to make the switch, and to what, before you leave the U.S., not after you land.
